And judgement. Alzheimer's disease AD ; is the commonest cause of dementia and is characterised by degeneration of specific nerve cells, and presence of neuritic plaques and neurofibrillary tangles in the brain. There are over 700, 000 people with dementia in the UK, of whom about 400, 000 have Alzheimer's disease. Incidence is highly age dependent affecting 5% of those aged over 65, and 25% of those over the age of 85. Annual costs of dementia care in the UK are estimated to be 6.1 billion. Cholinesterase inhibitors : The symptoms of Alzheimer's disease are believed to be at least partly due to low levels of acetylcholine in the brain because of degeneration of the nerve cells that produce this molecule. Cholinesterase inhibitors inhibit the enzymes responsible for metabolising acetylcholine, thereby enhancing neurotransmitter levels. Donepezil Aricept ; was the first cholinesterase inhibitor to be licensed in the UK, in March 1997, followed by rivastigmine Exelno ; and galantamine Reminyl ; . Donepezil is manufactured by Eisai Ltd. and marketed by Pfizer Ltd., but neither company was involved in the AD2000 study beyond supply of the drug on a commercial basis. Previous randomised trials, among patients with mild to moderate 'probable Alzheimer's disease', established that donepezil, and other cholinesterase inhibitors, produce small improvements in tests of mental ability, but with no evidence that the drugs reverse the underlying disease process. It was also not clear how the drugs affect day-to-day functioning, or behavioural disturbance, or the quality of life of the patient and their carer. It is estimated that total spending on cholinesterase inhibitors since the drugs were licensed in 1996 exceeds three billion pounds worldwide. The University of Birmingham Clinical Trials Unit This is a Research Unit within the School of Medicine specialising in the design, conduct and analysis of clinical trials. It commenced operation in 1997, with funding from the NHS research support budget, and builds on the strong clinical trials expertise existing at the University of Birmingham. Its main brief is to undertake high-quality clinical trial research across a wide range of diseases. bctu.bham.ac.
Would be very appreciated not only by the patients, but the caregivers." It can be hard to get Alzheimer's patients to swallow pills, and Dxelon can cause serious nausea and vomiting. Applied once a day, the new skin patch sends Exellon straight into the bloodstream, bypassing the gastrointestinal tract in hopes of fewer side effects and maintaining a consistent daylong dose. The lowdose patch was just as effective as the high-dose pills but pill users suffered three times more nausea and vomiting than patch users. The patch clearly improves patients' quality of life, said Dr. Sam Gandy, director of the Farber Institute for Neurosciences at Philadelphia's Thomas Jefferson University, and an Alzheimer's Association spokesman. About 4.5 million Americans have Alzheimer's, a toll expected to reach a staggering 14 million by 2050 with the graying of the population. It gradually robs sufferers of their memories and ability to care for themselves, eventually killing them. There is no known cure or prevention; today's drugs only temporarily treat symptoms. Exslon does that by inhibiting the breakdown of a brain chemical called acetylcholine, which is vital for nerve cells to communicate with each other. The longer acetylcholine sticks around, the longer those cells can call up memories. Hopes are for a better drugs that could slow Alzheimer's inevitably worsening brain decay. A chief target is a sticky protein called beta-amyloid that forms clumps that coat and probably kill brain cells.
Thrombotic thrombocytopenic purpura TTP ; is characterized by the pentad of microangiopathic haemolytic anaemia MAHA ; , thrombocytopenia, neurological symptoms, renal dysfunction and fever Moake & Chow, 1998 ; . Haemolytic uraemic syndrome HUS ; is a clinically similar disease in which renal dysfunction predominates. Severe deficiency of von Willebrand Factor-cleaving protease VWF-cp ; , resulting in the presence of very adhesive unusually large VWF multimers in plasma has been consistently found in patients with TTP, whereas VWF-cp was normal in patients diagnosed with HUS Furlan et al, 1997; Furlan et al, 1998a, b; Tsai & Lian, 1998 ; . Oleksowicz et al 1999 ; reported VWF-cp deficiency 15% ; in patients with metastasizing malignancies, whereas in patients with localized tumours VWF-cp activity was found to be normal 88% ; . To examine if, in patients with metastasizing tumours, MAHA might be explained by a deficiency of VWF-cp, we studied four patients with metastasizing neoplasia-associated MAHA. PATIENTS AND METHODS Patient A was a 77-year-old man with cancer of the prostate and disseminated bone metastases. Orchidectomy in 1996.

Fortunately, Maryland is a member of the PJM Interconnectio n that allows it to access resources in 13 other states to help meet the needs of Maryland consumers. The PJM market place helps Maryland meet its reliability needs by providing access to a pool of over 160, 000 megawatts. The installed reserve margin currently 15% ; reflects the ability of PJM to dispatch out-of-state generation during times of high demand in Maryland. Absent regional reliability benefits, Maryland, standing alone, would require a greater reserve margin to accommodate instate generation or transmission contingencies. The PJM market has a built- in mechanism to insure that an adequate supply of power is available to meet the needs of Maryland consumers. The Reliability Pricing Model RPM ; , PJM's newly- instituted capacity market construct, is specifically designed to encourage generation and demand response in the locations where it is needed most. Although RPM has been in effect for approximately one year, there is every reason to believe that it will lead to the necessary investments in new resources. Indeed, it is not a question of "waiting for the market to work" it already is. PJM has determined that the three transitional RPM auctions that have been held to date have added about 10, 000 MW to the grid that would not have been there without RPM. These additions include significant increases in demand response. 2 Specifically, a review of the PJM Interconnection queues reveal that since FERC approved the RPM settlement on December 21, 2006, 327 new projects representing nearly 70, 000 MWs have been submitted for analysis into the PJM queue process. Of these projects, roughly 300 projects, which would produce over 55, 000 MWs of new capacity, remain active and 18 of these projects totaling 2192 MWs ; are located in the state of Maryland. It is reasonable to expect that not all of these projects will be constructed; but the current level of interest in building new resources is evidenced by the activity in the PJM queues following the approval of RPM. This recent flood of activity is attributable to RPM and should not be minimized as coincidental. P3 members have been actively evaluating new projects since the advent of RPM and several have made public announcements about these proposed additions. Consider this quote from Exelob announcing a new 600 MW facility in the PJM footprint: "A key factor in the company's decision to pursue development opportunities now is the competitive market structure within PJM, Exelon Power President Mark Schiavoni said. Exelon Power is the non- nuclear generating unit of Exelon Generation.

The epidemiology and natural history of POAG are relatively well understood. Until recently the epidemiology of PACG was not as clearly understood, but over the last 5 years there has been an increased research effort, and this deficiency is gradually being re-dressed. Previously, IOP was held to be the most suitable risk-factor for POAG that could be used for screening. However, although raised IOP is sufficient to cause glaucoma, it is not necessary. Between one-half and two-thirds of POAG cases have an IOP consistently within the 'statistically normal' range. ~sychophysical tests and disc imaging techniques offer promise although the technology is immature and remains to be proven. Anterior chamber depth In contrast, PACG does have features that are more readily identifiable. Closure of the drainage angle requires the iris and the trabecular meshwork to be in relatively close proximity prior to the development of the closure process. The association and kytril.

Ms Crowe testified that while she was the nurse coordinator relative to the claimant's case, there was not a surgical recommendation made by either of the physicians to see and or treat the clamant. Ms. Crowe's testimony reflects that during a October 28, 2003, conversation with the claimant he relayed that he had a car accident "last week" as the reason he did not attend physical therapy. Claimant also relayed that he did not seek treatment from the emergency room following the accident. Ms. Crowe confirmed her contact with the claimant, as documented in her progress record, on December 15, 2003, when claimant return to work pursuant to the release of Dr. Aiken, and on December 24, 2003; January 5, 2004; January 12, 2004; and January 13, 2004. There was no other contact with the claimant by Ms. Crowe until May 25, 2004. Ms. Crowe explained the referral to Dr. Jordan and subsequent surgery, the third-party administrator recommended that she have no contact with claimant as nurse case manager. Ms. Crowe acknowledged that she is unaware of what claimant's condition was between the first of January and May 2004. Ms. Crowe further acknowledged that neither Dr. Meador, Dr. Pahls or Dr. Hart is a surgeon. The only surgeons involved in claimant's medical treatment relative to his May 2003 compensable injury prior to Dr. Jordan were Dr. Saer and Dr. Aiken. Dr. Saer last saw the claimant on July 3, 2003. Prior to returning to work in December 2003, claimant was contemplating a second opinion evaluation with Dr. Jordan. Dr. Hart deferred to Dr. Jordan relative to possible surgery regarding the claimant. The management of the claimant's medical treatment relative to the May 2003 injuries 12.

EXECUTIVE SUMMARYScope Datamonitor insight into the Alzheimer's disease marketAricept is the current gold standard treatment for AD. According to interviewed physicians, Pfizer Eisai's market leading drug has a better side effect profile than rival compounds, and is available in a convenient once-daily tablet, which aids compliance. In addition, Aricept comes in an easy two-step dosing scale. Nevertheless, physicians and opinion leaders believe the drug is no more effective at treating the disease than Novartis' Exelon and Janssen Shire's Reminyl. As well as current AD-approved drugs, numerous other drug therapies are given to patients to treat behavioral and psychological symptoms. SSRI antidepressants are the most commonly prescribed drugs for depression followed by atypical neuroleptics to treat psychosis, agitation and aggression. Considering that millions of patients suffer from these symptoms, Datamonitor believes the AD market represents a significant revenue generator for psychotropic drug manufacturers.Diagnosis rates vary significantly across the seven markets. Germany currently has the lowest rate of 39%, whilst Spain has the highest at 72%. Increasing awareness in the general public is the best way to improve these rates. Nevertheless, Datamonitor believes there is a strong need for novel diagnostic technologies that can accurately identify people at risk of developing the disease.According to interviewed physicians, between 30% and 44% of patients fail first line therapy across the seven markets, thus highlighting a distinct weakness of current drugs. However, with the expected launch of Merz Lundbeck's memantine, physicians will soon have an alternative treatment option with a different mechanism of action. According to interviewed opinion leaders, memantine is expected to receive rapid uptake for the moderate to severe group of AD patients, and will be used in conjunction with AChEIs.Key metrics TABLE OF CONTENTSCOMPARATIVE ANALYSISSummary of key findings across countriesEpidemiology of Alzheimer's disease across the seven major marketsIntroductionPrevalence of Alzheimer's diseaseBreakdown of disease stagesBreakdown of symptomsPresentation of Alzheimer's disease sufferers across the seven major marketsPhysician involvementReferral patternsDiagnosis and evaluation of Alzheimer's disease across the seven major marketsIntroductionDiagnostic tools and techniquesDiagnosis ratesTreatment of Alzheimer's disease across the seven major marketsOverviewTreatment ratesTreatment timelinesPhysician involvementNon-pharmacological therapyDrug treatment patternsFirst line treatment of different disease stagesTreatment patternsGold standard therapyTreatment switchingTreatment failureTreatment outcomeCo-morbidities and off-label prescriptions across the seven major marketsTreatment of co-morbiditiesOff-label prescriptions of current AD drugsDrug profiles across the seven major marketsMethodologyEfficacySide effectsOther criteriaFuture of the treatment of Alzheimer's diseaseUnmet needsFuture trendsTHE US MARKETExecutive summary The 2001 launch of Exelon and Reminyl on the US market has begun to erode Aricept's market share. Exelon Pension Claim. On July 11, 2006, a former employee of ComEd filed a purported class action lawsuit against the Exelon Corporation Cash Balance Pension Plan Plan ; in the Federal District Court for the Northern District of Illinois. The complaint alleges that the Plan, which covers certain management employees of Exelon's subsidiaries, calculated lump sum distributions in a manner that does not comply with the Employee Retirement Income Security Act ERISA ; . The plaintiff seeks compensatory relief from the Plan on behalf of participants who received lump sum distributions since 2001 and injunctive relief with respect to future lump sum distributions. It remains to be determined whether this case will proceed as a class action and how many Plan participants may be part of the proposed class, if a class is certified. However, the lawsuit is not expected to have a material financial impact on Exelon. Savings Plan Claim. On September 11, 2006, five individuals claiming to be participants in the Exelon Corporation Employee Savings Plan, Plan #003 Savings Plan ; , filed a putative class action lawsuit in the United States District Court for the Northern District of Illinois. The complaint names as defendants Exelon, its Director of Employee Benefit Plans and Programs, the Employee Savings Plan Investment Committee, the Compensation and the Risk Oversight Committees of Exelon's Board of Directors and members of those committees. The complaint alleges that the defendants breached fiduciary duties under ERISA by, among other things, permitting fees and expenses to be incurred by the Savings Plan that allegedly were unreasonable and for purposes other than to benefit the Savings Plan and participants, and failing to disclose purported "revenue sharing" arrangements among the Savings Plan's service providers. The plaintiffs seek declaratory, equitable and monetary relief on behalf of the Savings Plan and participants, including alleged investment losses. Approximately 90, 000 acre-feet of water per year will be needed for the operation of the Plant. GBRA currently has more than that amount of water available for supply at the Point of Take from GBRA's Canal System in Calhoun County, Texas, assuming certain improvements are made to the Canal System. Exelon desires that GBRA reserve for Exelon, for a defined period of time ending June 30, 2009, a water supply of 90, 000 acre-feet per year at the Point of Take from GBRA's Canal System. Exelon seeks such a reservation so that it will have the opportunity, during the defined period of time, to prepare or have GBRA prepare cost estimates, additional hydrologic studies to evaluate the firmness of the water supply, and other studies, and, if Exelon decides to pursue development of a Plant in Victoria County, to seek to negotiate, before June 30, 2009, the terms and conditions of a detailed, long term agreement with GBRA for the reservation and supply of such water at the Point of Take from GBRA's Canal System and the delivery of such water by GBRA from the Point of Take to the Plant. GBRA is willing to agree to reserve for Exelon such a water supply at the Point of Take from GBRA's Canal System, but only for the defined period of time ending June 30, 2009 in accordance with the terms and conditions set forth in the Reservation Agreement. Of the sale of a generating unit, including the buyer's desired business and tax structures, would determine which option would be utilized. Because there are likely to be multiple buyers of the Subject Assets each buyer a ``Third Party'' ; , the Applicants may utilize either of the disposition options described below to effectuate the sale of the Subject Assets to each Third Party the disposition to each Third Party is referred to as a ``Divestiture Transaction'' ; . Each of the Subject Assets would be acquired pursuant to each Divestiture Transaction in exchange for cash and or notes the ``Transfer Consideration'' ; . Option 1: Exelon Generation would sell each of the assets listed in Exhibit G13 of the Application to a Third Party pursuant to the Divestiture Transaction in exchange for the Transfer Consideration. Exelon Generation may distribute to Exelon, through Ventures, the Transfer Consideration received. Option 2: Exelon Generation would sell, in exchange for an amount of cash equal to the Transfer Consideration each of the assets listed in Exhibit G14 to the Application to the corporation wholly-owned by Ventures that is listed as the ``Acquiring Sub'' next to that asset in Exhibit G14. Exelon Generation may distribute to Exelon via Ventures ; the cash received. Ventures would then sell all of the interests in the Acquiring Sub to the Third Party in exchange for the Transfer Consideration. The particulars of the option selected for each Divestiture Transaction would be specified in the applicable PostMerger FERC Compliance Filing. Applicants state that each of the steps outlined in Option 2 could occur simultaneously. VIII. Section 1081 Recitals Applicants state that Internal Revenue Code section 1081 d ; provides for the nonrecognition of gain or loss from certain intercompany transactions between members of the same system group if such transactions are made in obedience to a Commission order. Applicants request that the order on this Application: i ; Recite that the sale or disposition of generating units as part of the Generation Transactions is necessary or appropriate to the integration or simplification of the postMerger Exelon holding company system and to effectuate the provisions of section 11 b and ii ; require postMerger Exelon to take appropriate actions to cause its direct and indirect subsidiaries, as the case may be, to complete the Generation Divestiture as required in order to comply with the Merger Order. IX. Affiliate Transactions A. Service Company Transactions Applicants state that, under the 2000 Merger Order, the Commission authorized Exelon to organize and capitalize Exelon BSC as a service company subsidiary, and authorized Exelon BSC to provide ComEd, PECO and other companies in the Exelon system with administrative, management, engineering, construction, environmental, and other support services pursuant to a General Services Agreement. Further, Exelon filed a post-effective amendment in File No. 709645 describing its accounting systems and cost allocation methodologies and request a supplemental order of the Commission, as required by the 2000 Merger Order. On October 31, 2003, Exelon submitted a 60-day letter that, as supplemented, described certain proposed changes in allocation methods for ``corporate governance costs, '' and the reorganization of Energy Delivery Shared Services, a business unit of Exelon BSC that would begin to provide new services to ComEd and PECO effective January 1, 2004.11 In connection with the Merger, the Applicants state that PSEG Services would sell all of its assets to Exelon BSC, change its name and remain as a subsidiary. Post-Merger, Exelon BSC intends to add the former PSEG companies as client companies under the General Services Agreement and would provide to the new client companies the same administrative, management, and technical services that it now provides to Exelon system companies, utilizing the same work order procedures and the same methods of allocating costs that are specified in the General Services Agreement.12 In connection with the Transaction, certain employees of PSEG Services may be transferred to and become employees of Exelon BSC, which would be the sole subsidiary service company for the Exelon system. Exelon requests that the Commission find, that Exelon BSC would continue to be organized and conducted in accordance with section 13 b ; of the Act. Applicants request authority to delay the full implementation of all services and systems relative to the new.
Sumption under increasing schedule requirements for more potent reinforcers. Finally, the slope of the line depicting consumption at each schedule value should be steeper for less potent reinforcers i.e., reinforcer consumption should be less stable ; than that for more potent reinforcers. Although these methods have been used infrequently in applied research, they would permit multiple evaluations of data generated from a rapid assessment of responding under PR schedules. Multiple analyses may be especially useful when a limited amount of data are available to identify functional relations Vollmer, Iwata, Zarcone, Smith, & Mazaleski, 1993 ; . In light of recent findings by Tustin 1994 ; and DeLeon et al. 1997 ; , additional research is warranted on methods for identifying effective reinforcers under increasing schedule requirements, as well as on the utility of these methods for developing effective behavior programs. Procedures from the basic laboratory i.e., PR schedules and behavioral economic analyses ; may be helpful for developing an applied technology in this area Mace & Wacker, 1994 ; . Thus, the purpose of the current study was to extend previous findings by determining whether PR schedules and behavioral economic data analysis methods are useful for differentiating among preferred stimuli in a relatively brief manner. In Experiment 1, the reinforcing efficacy of preferred stimuli was compared under PR schedules. To evaluate the utility of this assessment, the effectiveness of these reinforcers when incorporated into various treatments for destructive behavior was compared in Experiment 2. EXPERIMENT 1: REINFORCER ASSESSMENT METHOD Participants and Settings Four individuals with developmental disabilities participated in Experiment 1. All and atrovent.

6.2. Opiates and other analgesics 626. Influence of age on flurbiprofen axetil requirements for preventing pain on injection of propofol in japanese adult surgical patients: A prospective, randomized, double-blind, vehiclecontrolled, parallel-group, dose-ranging study - Fujii Y. and Nakayama M. [Dr. Y. Fujii, First Department of Anesthesiology, Toho University School of Medicine, Tokyo, Japan] - CLIN. THER. 2006 28 8 ; - summ in ENGL Background: Pain on injection is a recognized adverse event AE ; of propofol administration for the induction of general anesthesia. Preceded by venous occlusion, flurbiprofen axetil, a prodrug of the NSAID flurbiprofen, has been associated with a reduction in pain induced by propofol injection. A review of the literature determined that no published data were available on the influence of age on the requirements for flurbiprofen axetil dose. Objective: This study was undertaken to examine the influence of age on flurbiprofen axetil requirements for the treatment of prop ofol-induced pain on injection in Japanese adult surgical patients. Methods: This prospective, randomized, double-blind, vehicle-controlled, parallel-group, dose-ranging study was conducted at the Department of Anesthesiology, Institute of Clinical Medicine, University of Tsukuba, Tsukuba, Japan. Young age 20-40 years ; and older age 60-80 years ; Japanese patients scheduled for various types of surgery eg, cholecystectomy ; were eligible. Patients were randomized to receive flurbiprofen axetil at 1 of doses 25 or 50 mg, regardless of body weight ; or inactive vehicle saline ; , preceded by manu al venous occlusion with a rubber tourniquet for 2 minutes and followed by the infusion of the first 25% of calculated propofol dose 0.5 mg kg at room temperature, 23C ; into the largest dorsal vein of the hand through a 20-G IV cannula without local anesthesia ; , followed by the release of the occlusion and delivery of propofol 2 mg kg. An investigator blinded to treatment questioned each patient about pain intensity during propofol injection which was assessed using a verbal rating scale: 0 none; 1 mild; 2 moderate; and 3 severe. The overall prevalence of pain was calculated in each group. AEs at the injection site pain, edema, wheal, inflammation ; were assessed by the study inves tigator for 24 hours after surgery using spontaneous reporting and patient interview. Results: A total of 150 patients, 75 young adults 38 men, 37 women; mean [SD] age, 31 [5] years; mean [SD] height, 163 [7] cm; mean [SD] body weight, 58 [9] kg ; and 75 older adults 38 men, 37 women; mean [SD] age, 70 [6] years; mean [SD] height, 154 [8] cm; mean [SD] body weight, 53 [10] kg ; , were enrolled. Each treatment group comprised 25 patients. No differences in demographic characteristics were observed among the 3 study groups. In young patients, the overall prevalence of propofol-induced pain was significantly lower in patients who received flurbiprofen axetil 50 mg 12 [48%] ; compared with that in Section 30 vol 138.2 and combivent.

The largest increase in any country in 2005 was in Brazil, provisionally estimated at 4.4 million hectares 9.4 million hectares in 2005 compared with 5 million in 2004 ; , followed by the US 2.2 million hectares ; , Argentina 0.9 million hectares ; and India 0.8 million hectares ; . India had by far the largest year-on-year proportional increase, with almost a three-fold increase from 500, 000 hectares in 2004 to 1.3 million hectares in 2005. Biotech soybean continued to be the principal biotech crop in 2005, occupying 54.4 million hectares 60 per cent of global biotech area ; , followed by maize 21.2 million hectares at 24 per cent ; , cotton 9.8 million hectares at 11 per cent ; and canola 4.6 million hectares at 5 per cent of global biotech crop area ; . Herbicide Tolerence, Inset Resistance During the first decade, 1996 to 2005, herbicide tolerance has consistently been the dominant trait followed by insect resistance and stacked genes for the two traits. In 2005, herbicide tolerance, deployed in soybean, maize, canola and cotton occupied 71 per cent or 63.7 million hectares of the global biotech 90.0 million hectares, with 16.2 million hectares 18 per cent ; planted to Bt crops and 10.1 million hectares 11 per cent ; to the stacked genes. The latter was the fastest growing trait group between 2004 and 2005 at 49 per cent growth, compared with 9 per cent for herbicide tolerance and 4 per cent for insect resistance. Biotech crops were grown by approximately 8.5 million farmers in 21 countries in 2005, up from 8.25 million farmers in 17 countries in 2004. Notably, 90 per cent of the beneficiary farmers were resource-poor farmers from developing countries, whose increased incomes from biotech crops contributed to the alleviation of their poverty. In 2005, approximately 7.7 million poor subsistence farmers up from 7.5 million in 2004 ; benefited from biotech crops the majority in China with 6.4 million, 1 million in India, thousands in South Africa including mainly women Bt cotton farmers, more than 50, 000 in the Philippines, with the balance in the seven developing countries which grew biotech crops in 2005. This initial modest contribution of biotech crops to the Millennium Development Goal of reducing poverty by 50 per cent by 2015 is an important development which has enormous potential in the second decade of commercialization from 2006 to 2015.

Although often sponsored by companies, these public information campaigns do not mention a particular product or brand, but refer the consumer to their doctor or pharmacist for information and treatment, if warranted. Brand advertising It is argued that indirect advertising of schedule 3 products may be so indirect as to not adequately inform consumers about the product. For example, the Asthma Foundation observed that brand names are often the 'language of the market', and if they cannot be used, it is difficult to convey a meaningful message.

As I speak with members and potential members I often raise the question -- How are you using your membership in SAEM or, in fact, any organization? How many individuals have you known who sign up for a health club, attend regularly for a few weeks and then pay for the remainder of the year and seldom return? Membership in a professional Society has members who join for many different reasons. Just to name a few -- you want to publish in the journal; it provides an opportunity to network with others with similar interests; you want to give back to the profession; it may be a step toward participation in other organizations; it looks good on your CV; or most importantly you support the purpose mission of the organization even though currently you may not be able to actively participate. The list can go on but you can see that membership means different things to each person. I knew a mature physician serving on the nominating committee of an organization. At the end of the meeting he shared his thoughts when he said, "I don't know how each of you give so much time to the Society, I have tried and can't find the time. But I will always pay my dues and support your efforts to help my fellow physicians." How are you involved? How can you help with your limited time? One simple step is to encourage physicians in your department to join SAEM. Ask them about the benefit they receive from SAEM's support of emergency medicine research and tools to improve education of future EM physicians. Are residents using their membership? What would be helpful for them in the coming years, why should they remain members, even if they choose and buy kytril.

OPERATING EXPERIENCE RELATED TO POWER UPRATES Damage of Steam Dryers and Other Plant Components at Quad Cities and Dresden Exelon Generating Company, LLC Exelon ; , has discovered cracks in the steam dryer on three separate occasions at Quad Cities Unit 2 since the unit has operated at EPU power levels. Exelon also found cracks in the steam dryers at Dresden Units 2 and 3 and Quad Cities Unit 1. Flow-induced vibration contributed to failures of feedwater sampling probes at Dresden Units 2 and 3 and inoperability of an electromatic relief valve at Quad Cities Unit 1. Loose parts in the reactor coolant system have been generated from pieces of cracked steam dryers and flowinduced vibration damaged feedwater probes. The staff has determined that these issues do not pose an immediate safety concern given the current operating conditions at Quad Cities and Dresden. However, steam dryers and other internal main steam and feedwater components must maintain structural integrity to avoid generating loose parts that could impact safety system or reactor plant operation. Since 2002, steam dryer cracking and flow-induced vibration damage on components and supports for the main steam and feedwater lines have been observed at Dresden and Quad Cities following implementation of extended power uprates EPUs ; . In June 2002, approximately 3 months following implementation of a 17.8-percent EPU, Quad Cities Unit 2 experienced an increase in the moisture content of the steam flowing to the turbine. In July 2002, the licensee shut down Quad Cities, Unit 2, for inspection and identified cracks in the steam dryer. The licensee repaired the steam dryer, and returned the unit to power operation at the EPU power level. The steam dryer is not a safety-related component, but is required to maintain its structural integrity. Approximately 10 months following restart of Quad Cities, Unit 2 from the outage to repair the steam dryer, the plant experienced a similar increase in the moisture content of the steam. The licensee shut down the plant for inspection of the steam dryer and identified cracks in several locations of the steam dryer. On November 12, 2003, Quad Cities Unit 1 was shut down to perform inspections and repairs of the steam dryer. The unit had been operating at a reduced power level since November 3, 2003, due to indications of higher-than-normal moisture carryover in the reactor steam. On November 13, 2003, the steam dryer was found damaged during inspections following reactor disassembly. The damage occurred in the inch-thick upper dryer hood cover plate. The cover plate had cracks approximately 51 inches in total length and a 6 inch by 9 inch portion of the plate broke off from the steam dryer. Exelon conducted extensive inspections in an effort to locate the lost steam dryer piece s ; . The piece s ; were not recovered; however, Exelon has found indications on a recirculation pump impeller. Based on these indications, the material is most likely in the bottom of the reactor vessel. The licensee conducted a loose part analysis to determine potential effects on plant systems and concluded that it was safe to operate the plant with the loose part in the vessel. The staff reviewed the licensee's loose part analysis and agreed with the licensee. Repairs and modifications, similar to those completed on the Quad Cities Unit 2 steam dryer earlier in 2003, were also completed on Unit 1.

Question eQuery 22878 ; : Can we have a clear definition and intent in coding section P4 Devices and Restraints ; ? Is a resident's ability to physically remove a restraint taken into consideration? Response: The interRAI approved definition of restraints is "any manual method, or any physical or mechanical device, material, or equipment that is attached or adjacent to the resident's body that the resident cannot remove easily, and that does or has the potential to restrict the resident's freedom of movement or normal access to his or her body." If a restraint is used and the resident cannot remove it him- or herself, it would be considered a restraint. Question eQuery 28388 ; : If a resident is in a chair that prevents rising AND the chair also has a belt or restraint across the trunk, should this be coded as "chair prevents rising" or "trunk restraint, " or both? Response: If a resident is in a chair that prevents rising AND the chair also has a belt or restraint across the trunk, you would capture both "chair prevents rising" AND "trunk restraint" in section P4. Similarly, if a limb was restrained AND the chair prevented rising, you would capture both of those items. Question eQuery 22829 ; : Is a seat belt lap belt used on a cognitively impaired resident with spasms in order to prevent her from falling out of the chair considered a restraint in section P4 Restraints ; ? Response: If the resident is not able to remove the seatbelt lap belt him- or herself, it would be considered a restraint in section P4c Trunk restraint ; . The reason a restraint is used does not affect the coding.
To understand this it is helpful to look at fluids at a molecular level. Here fluid can be portrayed as layers of tiny balls having to slide past each other. So it should follow that the greater the length of the tube the larger the areas of adjacent `layers' of fluid and ultimately the greater the force higher pressures ; needed to move them. Fluid Viscosity: Using the previous explanation, the larger the molecules of a particular liquid the greater the resistance between molecules when trying to move them i.e. more viscous. Most fluids have the same approximation to water in terms of viscosity. The exception to this is glucose. So adding all of these individual pressures together will give you an idea of the total pressure needed to be overcome by the infusion device. Occlusions When occlusions occur several things happen: The patient will not receive the intended therapy The longer the occlusion continues the greater the pressure that develops in the line, as the pump continues pumping until the occlusion alarm limit is reached. Leakage of the drug may occur Infiltration or extravasation.

The North American market experienced a reawakening of M&A activity with bidder values up from US.4bn to US.5bn. Target values rose even more steeply, more than threefold from US.2bn to US.9bn. Even excluding the massive Exelon deal in 2004 and the largest deal in 2003, the aggregate value of targets in the region doubled year on year. Electricity deals dominated with only 12% of the total value of all North American power targets being accounted for by gas assets.
InfraSource. Operating revenues decreased 6 million at InfraSource due to the sale of the majority of the InfraSource businesses in the third quarter of 2003. For the remaining InfraSource businesses, operating revenues decreased 3 million as a result of the closing of certain businesses and the reduction of new business as a result of wind-down efforts and margin deterioration for these businesses. Exelon Services. Operating revenues decreased million at Exelon Services due to poor economic conditions in the construction market. This decrease was partially offset by improved performance contracting activities of million. Exelon Energy Company. Operating revenues increased million at Exelon Energy Company due to higher gas prices in 2003. In addition, customer growth in the gas and electric markets increased operating revenues by million and million, respectively. These increases were partially offset by the discontinuance of retail sales in the PJM region of million and the wind-down of the Northeast operations of million. Purchased Power and Fuel Expense. Purchased power and fuel expense increased primarily due to increased fuel costs at Exelon Energy Company due to higher gas prices and increased customer volume. Higher gas prices accounted for million of the overall increase and increases in customer growth in the gas and electric markets accounted for million and million, respectively. In addition, purchased power and fuel expense increased million from the impact of mark-to-market accounting. These increases were partially offset by reduced costs from the discontinuance of retail sales in the PJM region of million and the winddown of the Northeast operations of million.
The existing global Alzheimer disease treatment market reached an estimated value of .07 billion in 2005 and is forecast to reach .74 billion in 2010, a compound annual growth rate 20052010 ; of 7.1%. We expect this growth to come primarily through an expanded market volume, offset by the US patent expirations of two of the key brands--Razadyne patent expires in 2008 ; and Exelon patent expires in 2007 ; . The US market is expected to continue to dominate, reflecting this market's more aggressive approach to treatment of Alzheimer disease. In contrast, a more gradual, evidence-based uptake is expected in the European markets Source: Wood Mackenzie's Productview March 2006.

7. On January 9, 2006, Exelon filed a motion to intervene and comments in support of ComEd recovering a WDC. In this filing, Exelon argues that ComEd is entitled to charge transmission customers for use of its local distribution system, pursuant to Order No. 888-A, 7 and that Order No. 20038 does not require a separate wholesale distribution rate as part of the PJM OATT.9 Further, Exelon contends that the PJM market structure does not preclude ComEd from imposing a WDC. 8. Also on January 9, 2006, GSG filed a motion to intervene and protest. GSG contends that the PJM market design for interconnection is based upon interconnection charges that reflect incremental costs and benefits. GSG argues that the proposed WDC contained in the ISAs is, therefore, inconsistent with PJM's market structure and, moreover, is contrary to Order 2003's separation of interconnection service from delivery service. GSG additionally argues that Order No. 2003 requires a WDC to be included in an OATT and, finally, GSG insists that PJM's tariff does not provide for a WDC. 9. Exelon filed an Answer to the protest of GSG in which it states as follows: The ComEd distribution facilities at issue in this proceeding have never been subject to the PJM OATT or its predecessor ComEd OATT. These distribution facilities have up until now been used by ComEd to deliver electricity to retail customers under a statePromoting Wholesale Competition Through Open Access Non-Discriminatory Transmission Services by Public Utilities; Recovery of Stranded Costs by Public Utilities and Transmitting Utilities, Order No. 888, FERC Stats. & Regs. 31, 036 1996 ; , order on reh'g, Order No. 888-A, FERC Stats. & Regs. 31, 048 1997 order on reh'g, Order No. 888-B, 81 FERC 61, 248 1997 ; , order on reh'g, Order No. 888-C, 82 FERC 61, 046 1998 ; , aff'd in relevant part sub nom.
